Permissible Public Website Registration Form

If you purchase Bayer Consumer Health products from a Bayer Authorized Distributor for purposes of resale to end-user consumers and wish to sell Products on your own proprietary website, you must register your website with Bayer. Please note you are not authorized to sell Products on or through your own proprietary website until you have registered that website with Bayer.

To register your website, please fill out the Permissible Public Website Registration Form below, review the terms in the Bayer HealthCare LLC – Consumer Health Authorized Reseller Policy, including the attached Online Sales Guidelines, agree to the Legal Agreement, and click SUBMIT.

This Form is only used to register “Permissible Public Websites,” as defined in the Authorized Reseller Policy. The Authorized Reseller Policy prohibits you from selling Products on online marketplaces without Bayer’s prior written consent. Consent to sell Products on online marketplaces is not granted through this Form. This registration process and the Bayer HealthCare LLC – Consumer Health Authorized Reseller Policy are only applicable in the United States.

*Please review the restrictions on online sales described in the Bayer HealthCare LLC – Consumer Health Authorized Reseller Policy carefully. The Bayer HealthCare LLC – Consumer Health Authorized Reseller Policy prohibits Authorized Resellers from selling on third-party marketplace websites such as Amazon and eBay without Bayer’s prior written consent. Consent to sell on third-party marketplace websites is not granted through this registration process.

Legal Agreement

By checking this box, I agree, on behalf of the company, as follows:

1.Company shall abide by the Bayer HealthCare LLC – Consumer Health Authorized Reseller Policy.

2. Company understands that it is strictly prohibited from using Bayer’s name, logos, trademarks, copyrighted materials and any other intellectual property in any way other than that granted in the Bayer HealthCare LLC – Consumer Health Authorized Reseller Policy without Bayer’s express prior written consent in each instance.

3. Company affirms the information provided in this Permissible Public Website Registration Form is true, accurate, and current.

4. Company understands that any personal data shared with Bayer through this form will be handled in accordance with Bayer’s CH Privacy Statement
